Safe sleep risk assessment Template

This sleep and rest risk assessment template is designed to help approved providers and services conduct effective evaluations. It serves as a foundational tool that should be customised to address the specific needs of your service or Family Day Care (FDC), in accordance with regulations 84A, 84B, and 84C. It is mandatory for services and FDC  to perform a sleep and rest risk assessment at least once every 12 months, and promptly upon becoming aware of any factors that may impact the safety, health, and wellbeing of children during sleep and rest periods. This document is intended to be dynamic and should be regularly updated to reflect changing circumstances and emerging hazards.
